Job Description:

 

Title: Veteran Social Worker

Reports To: Director of Community Benefits Program

Location: Family Health Center at North General, New York, NY 10035

 

Summary: This is a grant funded, full time position in partnership with Single Stop USA and the Robin Hood Foundation. The Social Worker will provide clinical and case management/supportive social services to Veteran patients and their families in addressing personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care.

 

The Social Worker will collaborate with Institute for Family Health interdisciplinary treatment team in order to provide optimum patient-centered care. Clinical responsibilities include assessment, mental health screening, and Evidence Based treatments.

 

Case management responsibilities include eligibility screening for Benefit programs (Medicaid, Food Stamps, SSI, Housing services, etc.) and assistance in completion of Public Assistance applications. Social worker will develop linkages with area Veteran’s service organizations in promoting the program and facilitating patient’s receipt of services.

 

Candidate must be familiar with military culture and services; sensitive to concrete and psychosocial needs of Veterans and their families.

 

Candidate Requirements:

 

  • Master’s degree in Social Work, minimum.
  • MSW license or License Permit Required
  • Military Veteran from the service required
  • Experience and/or skills required:
  • Experience servicing Veteran population
  • Knowledge of Social Service agencies, HRA, and resources in the NY area
  • Healthcare experience preferred
  • Bi-lingual, Spanish preferred
  • Candidate MUST be familiar with military culture and services,
  • Sensitive to concrete and psychosocial needs of military veterans and their families.
  • Basic knowledge of computers, data entry experience a plus
  • Ability to write reports and correspondence
  • Ability to work independently and as part of a team
